DasCHE Spartans Criminal Background Check Consent Form

The safety of our children is always a main concern at DasCHE Spartans Sports Organization. Therefore, each and every adult who is a member of DasCHE Spartans will need to submit to a criminal background check.  This includes all leadership team members, coaches, assistant coaches, and volunteer workers.  Each and every adult who serves in our organization needs to submit his/her own information.
